When you initially contact us to set up an appointment, we will review your Medicare insurance benefits, your authorization needs for Workers Compensation, and/or your personal payment options. We will also discuss your condition and schedule an appointment to begin your plan of care.
At your first appointment, you will be asked to complete required forms and provide us with your Medicare and supplemental insurance cards and photo ID. You may wish to complete the required paper work prior to your first visit. Please click here for intake and financial forms.
On that first appointment you will undergo a detailed and comprehensive evaluation and begin a treatment program for your condition. During the evaluation, Karen will be taking a health history, performing appropriate tests and measurements and developing a treatment plan which includes your goals in your work with us. The evaluation and treatments run one hour per visit.
Finally, since it is our goal to provide you with continuity of care, Karen and Jilda will function as your team during the time you are working with us.
As a courtesy to you, we will only accept Medicare and Workers Compensation. We do not bill any other insurance companies, but are happy to provide the appropriate Superbill for you to submit to your own insurance company for reimbursement. Payment to us is expected at the time of your service. For your convenience we accept Visa, MasterCard and American Express.
Below are the forms that can be downloaded and brought to your first visit. Our office staff will assist you in determining which forms should be completed. If you have any difficulty completing the forms, we will be happy to assist you either on the phone prior to your appointment or at the time of your first appointment. We recommend, in order to safeguard your personal information, that you do not email these forms to us.
