Here’s a bit about what you can expect when you set up an appointment to work with us. When you call to make an appointment, we will begin by working with you to determine your insurance benefits, authorization needs and assign you the appropriate therapist for your condition. You may also find answers to your questions on our FAQ page.
When you come in to see us, you’ll be asked to complete any forms required, give us your insurance card and photo identification (for your protection), and asked if you have any questions regarding your therapy. On that first appointment you will undergo a detailed and comprehensive evaluation and begin a treatment program for your condition. During the evaluation, your therapist will be taking a health history, performing appropriate tests and measurements and developing a treatment plan which includes your goals in your work with us. The evaluation and treatments run one hour per visit.
Lastly, since it is our goal to provide you with continuity of care, we will assign you one primary therapist and a secondary therapist who will function as your team during the time you are working with us.
We accept all insurance plans and we are preferred providers for: Workers Compensation, Medicare, Blue Shield, Health Net, and The Marin IPA. If you are not covered by any of these plans, we offer flexible interest free payment plans, or we’ll gladly accept MasterCard, Visa or American Express.
In addition to making payment as convenient as possible, we offer the added benefit of flexible scheduling and extended hours.
Below are the forms that can be downloaded and brought to your first visit. Our office staff will assist you in determining which forms should be completed. If you have any difficulty completing the forms, we will be happy to assist you either on the phone prior to your appointment or at the time of your first appointment. We recommend, in order to safeguard your personal information, that you do not email these forms to us.
